: In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) - Bridgeport, CT

This full-time position supports high-priority business initiatives that require strong execution, sound judgment, and measurable follow-through. You will partner with cross-functional stakeholders to clarify priorities, solve operational challenges, and move important work forward, contributing to meaningful outcomes while strengthening systems within a healthcare environment.

Location: Remote – US-based candidates only; no visa sponsorship available.

Responsibilities

• Conduct in-home health evaluations to assess patients' health and needs.
• Engage patients in conversations to build trusted relationships.
• Review medical history and medications during visits.
• Administer diagnostic and preventive services as required.
• Travel locally and within licensed states for patient visits.
• Conduct virtual visits as needed.
• Utilize an iPad for intuitive workflow during evaluations.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.
• Experience in home health evaluations is a plus.
• Strong communication and interpersonal skills.

Pay

$95,738 – $206,206 annually.
