Job Summary and Qualifications

The Imaging Coordinator facilitates patient continuity of care within the imaging department. They assist with planning, organizing, implementing, and evaluating patient care needs by advocating, educating, and coordinating patients through their needs within the department. This role requires collaboration with local physician liaisons and leadership to conduct internal and external outreach and marketing.

What You Will Do in This Role

  • Assesses, plans, implements and evaluates patients according to physician orders, and establishes a plan of care in a timely manner.
  • Captures and uploads outside films and labs when applicable.
  • Coordinates 6 month follow up visits for applicable patients.
  • Schedules mammogram call back patients and patients requiring mammogram contrast studies.
  • Communicates with all members of the healthcare team who are providing patient care.
  • Schedules follow up studies as needed.
  • Facilitates timely access to quality medical care from receipt of physician order to diagnostic testing results.
  • Collaborates with physicians and other healthcare providers to facilitate process improvements within the interdisciplinary model.
  • Participates in quality improvement projects when applicable.

Qualifications

  • 1–2-year LPN/LVN diploma; ADN preferred.
  • Minimum of three (3) years of progressive hospital/clinical experience.
  • Experience in high-risk women’s program.
  • Experience in care coordination and/or patient education.
  • Experience in mammography and breast imaging modalities.
  • Experience in customer service or community engagement.
  • 1-5 years imaging experience.
  • LVN/LPN preferred; Registered Nurse (RN) preferred.
