IDT Coordinator
About the role
The IDT Coordinator at WelbeHealth plays a crucial role in supporting the interdisciplinary team (IDT) by coordinating their activities and providing administrative support. This position requires a high level of organization and attention to detail, as the coordinator serves as a key liaison between the IDT and various departments.
Responsibilities
- Serve as scribe for all care planning meetings and morning team meetings
- Track all service delivery requests, participants' hospitalizations, emergency room, and skilled nursing facility admissions and discharges
- Complete the monthly reassessment list to identify accountability for assessments and send reminders when assessments are due
- Collaborate with the Medical Records Coordinator and Quality Improvement team to upload care plans and signature pages into the EMR and perform audits
- Coordinate activities and provide administrative aid for the IDT including scheduling appointments, developing PowerPoint presentations, meeting agendas, generating reports, and conducting data entry as needed
Requirements
- High school diploma or equivalency
- Minimum of two (2) years of administrative support experience
- Experience using Microsoft Word, Excel, and PowerPoint

$23.23—$30.66 USD
