ICM Case Manager

Responsibilities

  • Carefully coordinates care activities and assists the individual as they move between and among services and supports.
  • Shares necessary care information with assigned parties such as the individual, Tier 1 and 2 Providers, specialty providers, residential provider, primary care physician, and other identified support.
  • Acts as a liaison for consumers within the agency and for other resources in the community.
  • Encourages and facilitates cooperation, pride, trust, and group identity; fosters commitment and team spirit; works cooperatively with others to achieve goals.
  • Collaborates with the general public, internal and external customers to provide information and quality services/products targeted to meet customer expectations.
  • Displays a high level of effort and commitment to performing work; operates effectively within the organizational structure; demonstrates trustworthiness and responsible behavior.
  • Attends training and seminars related to job duties as needed.

Requirements

  • A High School Diploma/GED Equivalent.
  • A valid Georgia Driver’s License.
  • Effective verbal and written communication skills.
  • Strong interpersonal skills and the ability to work effectively with diverse communities.
  • Experience with Microsoft 365 Office Products.
  • The ability to organize, prioritize and meet deadlines accordingly.

Qualifications

  • A Bachelor’s degree in a related field (psychology, social work, counseling, etc.) is preferred.
  • Previous case management experience is preferred.
