Responsibilities
- Carefully coordinates care activities and assists the individual as they move between and among services and supports.
- Shares necessary care information with assigned parties such as the individual, Tier 1 and 2 Providers, specialty providers, residential provider, primary care physician, and other identified support.
- Acts as a liaison for consumers within the agency and for other resources in the community.
- Encourages and facilitates cooperation, pride, trust, and group identity; fosters commitment and team spirit; works cooperatively with others to achieve goals.
- Collaborates with the general public, internal and external customers to provide information and quality services/products targeted to meet customer expectations.
- Displays a high level of effort and commitment to performing work; operates effectively within the organizational structure; demonstrates trustworthiness and responsible behavior.
- Attends training and seminars related to job duties as needed.
Requirements
- A High School Diploma/GED Equivalent.
- A valid Georgia Driver’s License.
- Effective verbal and written communication skills.
- Strong interpersonal skills and the ability to work effectively with diverse communities.
- Experience with Microsoft 365 Office Products.
- The ability to organize, prioritize and meet deadlines accordingly.
Qualifications
- A Bachelor’s degree in a related field (psychology, social work, counseling, etc.) is preferred.
- Previous case management experience is preferred.
