About the role

The Oregon Department of Human Services is hiring a Case Manager (Bilingual English & Vietnamese preferred) to assess Oregonians applying for, or currently receiving, long-term care services. In this role, you will develop, coordinate, and arrange services to allow older adults and people with disabilities to maintain continuity of care.

Responsibilities

  • Contact Oregonians or collateral contacts monthly to set up and monitor service plans and safety needs (in person, by phone, or electronically).
  • Collaborate with the individual, agencies, medical and care providers, and community partners to deliver benefits, services, and information.
  • Create person-centered risk mitigation plans and provide risk mitigation as needed.
  • Interview Oregonians to determine initial and ongoing eligibility for healthcare, social services, and protective services, including collateral interviews.
  • Develop detailed, individualized, and person-centered case plans.
  • Inform Oregonians of their rights, responsibilities, service options, and methods of recourse.
  • Utilize all applicable forms, computer systems, tools, and resources.

Requirements

  • Valid driver’s license and acceptable driving record.
  • One of the following:
    • Bachelor’s degree in Behavioral Science, Social Science, or a closely related field.
    • Bachelor’s degree in any field and one year of human services–related experience.
    • Associate degree in Behavioral Science, Social Science, or a closely related field and two years of human services–related experience.
    • Three years of human services–related experience.

    Human services–related experience includes work providing assistance to individuals and groups with issues such as economic disadvantage, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or inadequate housing.

Skills

  • Experience coordinating services and support by assessing needs, developing service plans, monitoring progress, and adjusting services to promote positive outcomes.
  • Experience in a fast-paced environment delivering customer service.
  • Experience interpreting and implementing policies and procedures.
  • Experience preparing and maintaining written narrative documentation.
  • Experience interviewing to obtain information and evaluate next steps in the intake process.
  • Bilingual English & Vietnamese preferred.

Working Conditions

  • Hybrid work is not available during trial service; may be discussed after trial service completion.
  • Flexible schedule with options to work between 6:00 AM and 6:00 PM.
  • Fast-paced environment with constantly evolving policies and procedures.
  • May drive a variety of vehicles to remote locations in diverse terrain and inclement weather.
  • Repetitive motions, prolonged sitting, and daily computer/technology use.
  • May encounter animals (some potentially dangerous) and be exposed to communicable diseases (e.g., flu, COVID-19, MRSA).
  • Interact with individuals who may have experienced trauma and exhibit emotional challenges.
  • Stressful situations requiring quick decision-making to ensure safety.

Background Checks

  • Criminal history and background check required for finalists; adverse results may lead to disqualification.
  • No visa sponsorship offered.
  • Must complete the US Department of Homeland Security’s I-9 form within three days of hire; E-Verify will confirm work authorization.

Pay

$4,519 – $6,904 per month.

Schedule

Permanent, full-time position represented by the Service Employees International Union (SEIU). This recruitment may be used to fill future vacancies in the same classification.
