About the role
This position provides medical case management to persons infected and affected by HIV/AIDS within the four-county area of Indian River, Martin, Okeechobee, and St. Lucie Counties (area 15).
Responsibilities
- Completes comprehensive Needs Assessments of individuals presenting themselves for assistance through the agency.
- Face-to-face assessment and re-assessment (including assessment of adherence to treatment, adherence to appointments, and family planning needs).
- Completes acuity assessment at the time of the initial comprehensive needs assessment.
- Develops an individualized care plan to address identified needs.
- Ensures that any applicable hierarchy of payer sources is used appropriately.
- Completes and maintains accurate documentation of all client-specific activities.
- Counsels, supports, educates clients on HIV, its transmission and complications, and HIV risk reduction.
- Links, refers, navigates, and retains clients in care.
- Participates in conference calls, staff, and planning meetings regarding Ryan White Case Management duties.
- Completes DOH and Ryan White medical Case Management mandatory trainings/webinars.
- Completes EARS/CSRS and timesheets within required timeframes.
- Completes Acuity Assessment including the following areas: Basic needs, co-morbidities, dental, family planning, health insurance/medical care coverage, HIV disease state, housing/living arrangements, mental health, risk behaviors, self-sufficiency, substance use, transportation, treatment adherence.
Requirements
- One year experience at a social service-related agency or other social services work position.
- Must be very detail-oriented and capable of thinking independently while exercising good judgment.
- Ability to work with a diverse population.
- Ability to prepare correspondence and administrative reports.
- Ability to work independently or with minimal supervision.
- Ability to plan, organize and coordinate work assignments.
- Ability to communicate effectively verbally and in writing.
- Ability to prioritize individual workload.
Qualifications
- Minimum: One year experience at a social service-related agency or other social services work position.
- Preferred: Bilingual (English/Spanish) / (English/Creole) or Tri-Lingual (English/Creole/Spanish).
