About the role

The Housing Supportive Services Program (HSSP) is a specialized program providing comprehensive and intensive field-based community mental health services to individuals in Permanent Supportive Housing (PSH) with Severe Mental Illness (SMI) or Serious Emotional Disturbance (SED).

Responsibilities

• Provide, coordinate, and implement mental health and recovery support services with individual members
• Conduct individual, family and group psychotherapy
• Develop effective, trusting relationships with members, focusing on facilitating independence and improved physical and mental health
• Collaborate with members to develop their individual service plans; review and update them upon goal completion
• Aid management and other clinicians with program evaluation and modification for future improvements and growth
• Absorb members into daily activities, socialization, health maintenance, sense of community, and recovery progress reflective of The People Concern's model emphasizing customer choice, harm reduction, and psycho-social rehabilitation
• Cooperate and collaborate with the HSSP team, participating in weekly staff meetings, group clinical supervision, and trainings
• Develop and maintain a complete, accurate, and current member file with all funder-required documents

Qualifications

• Master’s degree in Social Work, Psychology, or a related field from an accredited university
• Valid registration or licensure with the California Board of Behavioral Sciences
• Current, valid California Driver’s license with an acceptable driving record and reliable vehicle
• Able to transport members in your personal vehicle
• Outstanding clinical and strong team-building skills
• Detail-oriented with excellent time management, organizational, written, verbal, and computer skills
