About the role

The Housing Supportive Services Program (HSSP) is a specialized program providing comprehensive and intensive field-based community mental health services to individuals in Permanent Supportive Housing (PSH) with Severe Mental Illness (SMI) or Serious Emotional Disturbance (SED).

Responsibilities

• Provide, coordinate, and implement mental health and recovery support services with individual members
• Conduct individual, family and group psychotherapy
• Develop effective, trusting relationships with members, focusing on independence and improved physical and mental health
• Collaborate with members to develop and update their individual service plans
• Aid in program evaluation and modification for future improvements
• Assist members in daily activities, socialization, health maintenance, and recovery
• Engage in teamwork with other members of the HSSP team, including participating in staff meetings, clinical supervision, and trainings
• Develop and maintain a complete, accurate, and current member file with all required documents

Qualifications

• Master’s degree in Social Work, Psychology, or a related field from an accredited university
• Valid registration or licensure with the California Board of Behavioral Sciences
• Current, valid California Driver’s license with an acceptable driving record and reliable vehicle
• Able to transport members in your personal vehicle
• Outstanding clinical and strong team-building skills
• Detail-oriented with excellent time management, organizational, written, verbal, and computer skills

Work Environment

Combination of field and office environment, regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs. On occasion walk or drive to different local sites. Will necessitate working in busy and loud environments. Will be exposed to elements like cold, heat, dust, noise and odor. May need to bend, stoop, twist, and sit throughout the day.
