About the Role

Works in close collaboration with internal and external service providers to assess client needs, develop and implement service delivery aimed at maximizing independent functioning and housing stability in the community. Provides housing-related services including connecting with community resources, coordinating and monitoring services with other agencies, and monitoring lease compliance.

Responsibilities

  • Ensure all clients are treated with dignity and respect, experiencing effective yet minimally restrictive interventions.
  • Meet or exceed the organization’s access standards for timely service and departmental goals.
  • Ensure each client receives the highest level and quality of services available.
  • Actively research and engage in identifying internal and external resources and benefits advantageous to clients.
  • Ensure clients are appropriately connected to resources necessary for accomplishing their treatment plans and outcomes.
  • Develop and maintain respectful, courteous, and professional relationships with clients, their families, and all involved in service delivery.
  • Maintain positive, effective working relationships with staff, volunteers, donors, vendors, and other internal resources.
  • Develop and expand positive working relationships with referral sources and external partners.
  • Uphold and ensure compliance with internal policies, relevant laws, and regulations.
  • Model the values and mission of the organization and act as good stewards of its resources.
  • Engage in professional development activities to remain current with best practices.
  • Establish trusting relationships with clients, conveying hope and respect.
  • Ensure clients in permanent supportive housing maintain a clean, safe home through regular contact, encouragement, and life-skill training.
  • Work closely with clients to ensure understanding of lease obligations to promote housing retention.
  • Maintain a personal presence at housing sites to promote health, safety, and client participation.
  • Perform weekly apartment inspections using an inspection tool.
  • Collaborate with housing partners and landlords to promote housing stability and fulfill grant requirements.
  • Suggest other agency resources (peer support, vocational services, outpatient services) for referral and connection.
  • Use motivational interviewing techniques to actively engage clients in services.
  • Identify strengths and needs of clients and involve them in recovery in collaboration with the case management team.
  • Work to fully integrate clients into the community.
  • Provide education and support to clients' families.
  • Assist clients in supportive housing to foster a sense of community.
  • Provide individual/group psychosocial education related to clients' needs and interests.
  • Complete all required documentation thoroughly, on time, and according to agency standards.
  • Provide crisis prevention and intervention as needed in collaboration with the case management team.
  • Collaborate with the behavioral health treatment team and all internal/external service providers assisting assigned clients.
  • Support the Program Manager in collecting all required program documentation.
  • Uphold confidentiality and ensure compliance with landlord laws and federal regulations.

Requirements

  • Bachelor’s degree in human services (social work, psychology, rehabilitation, or related field) or four years of equivalent experience in community mental health.
  • Prior experience working with diverse populations, specifically adults with severe and persistent mental illness, people with substance use disorders, and people who are homeless or formerly homeless.
  • Ability to establish relationships with individuals and organizations within the social services community.
  • Demonstrated interpersonal and communication skills.

Qualifications

  • Valid driver’s license and full coverage insurance.
  • Ability to work in a stressful environment, including availability to respond to crisis situations at all times (nights, weekends, holidays).
  • Ability to effectively use all required devices, equipment, and computer systems.
  • Must meet all relevant licensing, credentialing, and other requirements.
