About the role

The Housing Case Manager (HCM) provides comprehensive case management services and care coordination to a caseload of residents living with HIV and/or co-occurring conditions. The HCM works closely with clients in their homes, the office, and the community to ensure housing stability by promoting independent living skills, financial management, and health and wellness.

Responsibilities

  • Provide case management services, including supportive counseling, motivational interviewing, crisis intervention, care coordination, linkage and referral, assessment, and service plan development.
  • Conduct field work and assess/observe residents in their living space at least once per month.
  • Document all services and interventions per funder, regulatory, industry, and internal standards.
  • Provide referrals and follow-up for primary care, substance use services, nutrition, clothing, mental health services, and financial management.
  • Maintain meaningful engagement with residents using motivational interviewing to help them achieve independent functioning.
  • Promote participation in community activities to foster individual growth and social support networks.
  • Coordinate with internal departments, including ADHC, Health Services, OASAS, Health Homes, Behavioral Health, Community Partnerships, Legal, and Job-Training.
  • Assist new clients with moving into apartments and transitioning from homelessness to stable housing.
  • Develop strength-based individualized goals in collaboration with clients and provide constructive feedback on goal participation.
  • Accompany residents to facilitate access to external services as needed.
  • Assess and discuss financial management, including rent obligations, utility payments, and nutritional needs.
  • Complete apartment inspections, place work orders, and collaborate with custodial and operations staff.
  • Actively participate in weekly supervision, prepared and engaged.
  • Complete all required reports and paperwork by deadlines.
  • Demonstrate excellent customer service with clients, staff, vendors, and stakeholders.
  • Coordinate care, including case conferencing, with internal and external service providers as part of a multidisciplinary team.
  • Perform other duties as assigned.
  • Participate in advocacy and direct actions to end homelessness and HIV/AIDS crises.

Key Performance Indicators

  • Successfully complete monthly home visits for 100% of clients.
  • Ensure HIPAA forms and income verification documents are uploaded and signed in each client’s file.
  • Create comprehensive initial assessments and service plans within 30 days of enrollment, with reassessments every 90 days.

Requirements

  • Bachelor’s degree in social work, psychology, counseling, or a related field preferred; Master’s degree a plus.
  • Minimum of three years of relevant experience in a social services setting working with individuals who have experienced homelessness, mental health issues, and/or substance use.

Qualifications

  • Ability to prioritize and multitask.
  • Experience and commitment to working with low-income and marginalized communities, including individuals with substance use and chronic medical/behavioral health conditions.
  • Ability to document client services in various electronic systems.
  • Dependable, flexible, and able to maintain client confidentiality.
  • Excellent customer service and interpersonal skills.
  • Strong verbal and written communication skills, including the ability to communicate with all levels of management.
  • Proficient in Microsoft Outlook, Word, and Excel.
  • Ability to collaborate with colleagues to solve problems.
  • Familiarity with Harm Reduction practices.

Working Conditions

  • Position requires 100% on-site presence at the program location.
  • Working knowledge of NYC mass transportation systems (subway and bus) to visit client apartments and meet clients; Metro Card provided.
  • Occasionally expected to de-escalate tense or hostile client situations.
