Qualifications
High School Diploma/GED required: BA degree preferred. Must have two years experience working with people who have developmental disabilities and one year supervisory experience required. A valid NYS driver’s license is mandatory and must be insurable under New Hopes’s insurance regulations. Possession and maintenance of certification to administer medication is mandatory.
Essential Functions
- Meet with supervisor regularly.
- Responsible for the day-to-day operations of the house and implementation of agency mission and goals.
- Obtain/Maintain required certifications as outlined by Administration/Staff Development and Training.
- Meet with the Service Coordinator, Nurse and Clinician on-going for both the coordination of the program planning and health needs of the individuals in the house.
- Serve as an acceptable role model for the individuals within the house and will be held to a higher standard. Follow all agency policies and procedures to include teaching, monitoring and ensuring compliance.
- Supervise and regularly communicate with and complete performance appraisals of all assigned staff.
- Coordination and implementation of individual’s objectives as outlined in their individualized plans.
- Oversee the organization and maintenance of the house and ensure the follow-up of all maintenance requests.
- Teach, train and educate all new staff in the functions of the house and provide ongoing training to existing staff including but not limited to abuse/mistreatment prevention training.
- Responsible for the development of the staff schedule to ensure adequate coverage and adequate supervision on all shifts.
- Ensure that all assessments appropriate for residential supports are completed in a timely manner.
- Responsible for record keeping as follows: Ensure necessary logs are complete and accurate. Maintain up-to-date clinical and medical files in conjunction with Nurse and Service Coordinator. Ensure accurate financial records are maintained for each individual. Ensure incident reports are completed, reviewed, follow-up and filed within the individual’s record. Ensure fire drills are conducted and maintain all fire safety documentation as required. Ensure that a mileage log and maintenance records are maintained for the house vehicles. Maintain data collection, goal sheets and attendance records. Supply inventories (i.e., house needs, clothing, hygiene supplies and personal possessions for all individuals).
- Be on call 24 hours per day in the event of emergencies.
- Responsible for house petty cash, food stamps, individual petty cash and all money within the house ensuring that receipts and accurate balances are maintained.
- Ensure appropriate menu planning in conjunction with the Nurse and Dietician.
- Supervise and ensure that all medication is ordered and delivered from the pharmacy. All medication is to be reviewed, including review of medication logs and checking the medication lock box.
- Authority to hire and fire employees and makes recommendations regarding hiring, firing and advancement (such as reclassifications and promotions)
- Maintain professional conduct when dealing with other agencies, families and community programs.
- Perform hands-on direct care responsibilities as needed, which include various types of physical intervention, i.e., lifting or transferring of individuals. Conduct investigations as assigned by the Director of Corporate Compliance and meet as required with the Special Review Committee. Provide initial 30-day and quarterly or semi-annual summary updates as applicable. Maintain required trainings and documentation as required on the various systems and logs provided
Additional Functions
- Coordinate all shopping trips.
- Ensure that individuals are neatly dressed and groomed.
- Provide staffing coverage as program indicates.
- Ensure that recreational activities are scheduled and that goals are followed through.
- Ensure all staff maintains required trainings within required time line.
- Supervise light maintenance, including but not limited to: minor household repairs, snow removal, lawn care, vehicle care and maintenance and to keep the house neat and clean through the use of preventative maintenance checklist.
- Keep all individual records and evaluations confidential.
- Ensure that medical and clinical appointments are scheduled when necessary and appropriate follow-up occurs.
- Plan and coordinate vacations for individuals.
- Participate in the staff in-service training programs as required.
- Carry out all other responsibilities deemed necessary to the position by the Assistant Director of Residential Services/Director of Residential Services.
Working Conditions/Physical Requirements
- Occasional sitting, pushing, pulling, bending and squatting.
- Occasional need to lift up to 50 lbs independently and up to 100 lbs with assistance.
- Occasional need for physical intervention (SCIP).
- Potential for exposure to blood and body fluids.
- Frequent driving.
