About the role

StoneGate Senior Living in Lewisville, TX, has opportunities for a Hospital Liaison in the Irving area to join our team at a facility that specializes in rehabilitative care. The Hospital Liaison serves as the primary contact with hospital patient referral sources and, in many cases, is the initial point of contact for potential admissions to the skilled nursing facility (SNF). You will play a key role in creating a positive resident experience by providing quality compassionate healthcare.

Responsibilities

• Assisting referral sources in identifying and assessing hospital patients with care needs that match the services provided by the SNF
• Educating and selling the potential patient and family regarding the SNFs' services, admissions process, and treatment process
• Communicating with appropriate SNF staff to ensure appropriate admission of new patients
• Ensuring that facilities achieve monthly budgeted census
• Assisting referral sources in identifying patients with care needs that match the services provided by the Hospital
• Completing preliminary, standardized assessments on each new patient referral
• Educating the potential patient and family regarding services provided, admissions process, treatment process, and projected outcome
• Communicating with appropriate nursing facility staff and ensuring accommodations and staff are prepared for patient arrivals
• Coordinating patient transfers and necessary transfers and admissions paperwork

Qualifications

• Must have 3 years of successful, high volume SNF admission/marketing experience in DFW
• Ability to communicate with all levels of personnel
• Must have excellent assessment skills
