Hospital Liaison

About the role

The Liaison will interact daily with hospital case managers at each assigned facility to deliver and set up O2 portability, nebulizers, and small DME items for patient discharge. The Liaison will provide education on insurance criteria, documentation requirements, equipment, and programs available to benefit patients and facilitate discharge.

Responsibilities

• Stock vehicle daily with necessary equipment, disposables, patient setup paperwork, and marketing literature needed for the day.
• Make rounds to contact all case managers each day at each assigned facility to notify them of your presence should they identify a patient discharge equipment need.
• Communicate with the center throughout the day regarding O2 discharges so the center can schedule a Service Representative to complete home setup of equipment.
• Understand and communicate coverage criteria to case managers and patients as applicable.
• Complete setup paperwork daily for all equipment delivered.
• Assist with obtaining CMNs and chart notes as requested by the center daily, required for billing.
