Hospital Liaison / Hospice Resource Nurse (RN)

About the Role

The Registered Nurse (RN) assesses hospice eligibility, provides education to patients and families, and collaborates with an interdisciplinary team to deliver high-quality hospice care.

Responsibilities

• Meet with potential patients and families to determine eligibility and provide hospice education in hospital settings.
• Conduct psychosocial assessments, develop interventions, and utilize community resources.
• Collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Manage patient care independently through communication, documentation, meetings, and adherence to hospice regulations.
• Maintain time management skills to handle visits, urgent needs, and meetings efficiently.
• Travel across the service area using reliable transportation, with a valid driver’s license and auto insurance.

Requirements

• Current RN licensure in good standing in relevant states.
• Ability to work effectively in team environments and independently.
• Strong communication, time management, and organizational skills.
• Willingness to learn electronic medical records systems and adhere to hospice rules.
• Reliable transportation for travel within the service area.
