RN Hospice Hospital Liaison

Skill Summary

• Active New Jersey RN license (BSN preferred)
• Excellent clinical skills with experience in hospice and/or community health nursing
• Experience conducting hospice or homecare admissions
• Exceptional verbal and written communication abilities
• Excellent organizational, customer service, and interpersonal skills
• Strong computer proficiency, including MS Office Suite

Responsibilities

• Support the organization’s mission by striving for excellence and fostering positive interpersonal relationships
• Assess patient needs and provide counseling to patients and families during the hospice referral and admission process
• Conduct comprehensive hospice admissions, including clinical assessments, eligibility verification, and documentation
• Cover the following hospitals: Newark Beth Israel Medical Center, Clara Maass Medical Center, Cooperman Barnabas Medical Center, and Jersey City Medical Center
• Coordinate transition to hospice services to promote continuity of care
• Maintain liaison with physicians, external partners, and internal care teams
• Monitor and support quality and appropriateness of services and programs
• Conduct community outreach to enhance hospice awareness and accessibility
• Assist in the development and implementation of a hospital discharge program at Community Medical Center
• Adhere to agency infection control and safety protocols and organizational policies

Requirements

• Active New Jersey RN license
• BSN preferred

Pay

• USD $43.00 to USD $51.00 per hour
