Registered Nurse Hospice Liaison Location: Middlesex County Schedule: Full Time Position Summary: The VNA Health Group is seeking a compassionate and experienced Full Time Registered Nurse Hospice Liaison to join our dedicated Hospice Team. This vital role focuses on assessing patients for hospice eligibility, coordinating their transition to hospice care, and conducting patient admissions. As a frontline advocate for hospice services, you will foster continuity, collaboration, and trust among patients, families, and referring partners—all while supporting our mission of delivering patient- and family-centered care. Position Highlights: Serve as a bridge between healthcare providers and hospice patients to ensure compassionate, coordinated transitions of careConduct in-home and facility-based hospice admissions with clinical accuracy and sensitivityEngage with the community and clinical partners to increase access and awareness of hospice careCollaborate with a team of interdisciplinary professionals committed to quality and comfort What You Will Do: Support the organization’s mission by striving for excellence and fostering positive interpersonal relationshipsAssess patient needs and provide counseling to patients and families during the hospice referral and admission processConduct comprehensive hospice admissions, including clinical assessments, eligibility verification, and documentationCoordinate transition to hospice services to promote continuity of careEstablish and maintain liaison with physicians, external partners, and internal care teamsMonitor and support quality and appropriateness of services and programsConduct community outreach to enhance hospice awareness and accessibilityAssist in the development and implementation of a hospital discharge program at Community Medical CenterAdhere to agency infection control and safety protocols and organizational policies What You Will Need: Bachelor’s degree in Nursing requiredActive New Jersey RN licenseExcellent clinical skills with experience in hospice and/or community health nursingExperience conducting hospice or homecare admissions strongly preferredExceptional verbal and written communication abilitiesExcellent organizational, customer service, and interpersonal skillsStrong computer proficiency, including MS Office SuiteValid NJ driver’s license, personal vehicle, and proof of insurance What We Offer: Health, dental, and vision insurance403B and 401a retirement plansTuition reimbursement and continuing education programsRobust clinical support and leadership development pathwaysEmployee referral and recognition programsPublic Service Loan Forgiveness Partner Working Conditions/Physical Demand: Fast-paced healthcare environment with frequent phone, scanner, email, and computer use. In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $42.00 to USD $50.00.
