Hospice Resource Nurse, RN

About the role

The Resource Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources.

Responsibilities

• Provide care and services for a group of patients across various settings (private homes, Skilled Nursing Facilities, assisted living facilities)
• Collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership as part of an interdisciplinary team
• Work closely with patients on a 1-1 level with strong support from a patient care team
• Work independently on day-to-day activities including phone, email, and written communication, meetings with healthcare professionals
• Manage visits, documentation, and meetings by prioritizing and triaging multiple concerns
• Travel across the service area to visit patients

Requirements

• Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice
• Willingness to drive with reliable transportation, valid driver's license, and auto insurance
• Ability to travel across the service area

Qualifications

• Works well within a team environment
• Ability to work independently
• Understanding of hospice rules and regulations
• Time management skills to manage visits, documentation, and meetings
• Ability to learn an electronic medical records system

Pay

Competitive Pay salary range $75k - $85k BOE
