Responsibilities

• Provide care and services for a group of patients, visiting patients where they reside (private homes, Skilled Nursing Facilities, and assisted living facilities)
• Serve as a contributing member of the interdisciplinary team, providing expertise regarding psych‑social assessments, interventions, and knowledge of community sources

Requirements

• Current Registered Nurse (RN) licensure in good standing in the state(s) of practice
• Ability to work well within a team environment, collaborating daily with nurses, social workers, chaplains, volunteers, and CNA leadership
• Ability to work closely with patients one‑on‑one while receiving strong support from a patient care team
• Ability to work independently on a day‑to‑day basis, including phone, email, and written communication, meetings with healthcare professionals, and understanding of hospice rules and regulations
• Time management skills to manage visits, documentation, meetings, and respond to urgent needs by prioritizing and triaging multiple concerns
• Ability to learn an electronic medical records system
• Willingness to drive with reliable transportation, valid driver’s license, and auto insurance to travel across the service area

Schedule

Monday–Friday, 3:00 PM – 11:00 PM (shift negotiable within reason)

Pay

Salary range: $75,000 – $85,000 per year, based on experience (BOE)
