Hospice Liaison Nurse - Registered Nurse, RN

About the Role

The Hospice Liaison Nurse clinically assesses and admits patients to Hospice care, facilitates discharge planning from the acute inpatient setting, and coordinates care and discharge planning with healthcare partners, facilities, patients, and their loved ones.

Responsibilities

• Deliver remarkable patient experience in every dimension, every time.
• Perform nursing assessments and apply Hospice philosophy in patient care.
• Interpret and obtain information to appropriately receive referrals for Hospice services.
• Determine patient eligibility for admission based on Medicare/Medicaid rules and regulations.
• Apply knowledge of nursing and pharmacy procedures specific to infusion and enteral therapies.
• Utilize strong writing, organizational, and analytical skills.
• Interact effectively with diverse individuals at all levels of authority, in teams, and on committees.
• Handle distractions, interruptions, emergencies, and unpredictable situations.
• Be available for after-hours call on a rotational basis.

Requirements

• High School Diploma or GED (required).
• 4-Year/Bachelor’s Degree (preferred).
• Graduate of an accredited school of nursing (required); BSN (preferred).
• Minimum three years’ experience in home care or hospice (required).
• Previous experience in managed care, infusion care, case management, and home health/hospice case management (preferred).
• Current RN licensure in the appropriate state (required).
• Reliable transportation for work-related assignments and activities.
• Flexibility to travel between locations in varying weather and road conditions.
• Computer experience.

Skills

• Excellent nursing assessment skills and clinical judgment.
• Understanding of Hospice philosophy and services.
• Knowledge of Medicare/Medicaid eligibility criteria.
• Strong writing, organizational, and analytical skills.
• Flexibility, adaptability, and problem-solving abilities.
• Ability to work in distracting environments (e.g., noise, telephone calls).
• Specific vision abilities: close vision, distance vision, night driving, and ability to adjust focus.
