Hospital Liaison

About the role

Autumn Lake Healthcare is seeking an experienced and motivated Hospital Liaison to build strong relationships with hospitals, healthcare providers, patients, and families, ensuring a smooth transition into our skilled nursing facilities. We provide our staff with the resources, tools, and support needed to succeed and grow in a caring, compassionate, and team-oriented environment.

Responsibilities

• Develop and maintain strong relationships with hospitals, discharge planners, case managers, and other healthcare professionals
• Serve as a key point of contact for hospital referrals and potential admissions
• Assist with coordinating a smooth transition from the hospital to the appropriate skilled nursing facility
• Educate referral sources, patients, and families on available services and care options
• Collaborate with facility leadership and admissions teams to support census growth and referral development
• Maintain consistent communication and follow-up with referral partners

Requirements

• Previous experience as a Hospital Liaison preferred
• Prior healthcare experience required
• Strong communication and relationship-building skills
• Ability to work independently while collaborating with multiple facilities and healthcare partners
• Strong organizational and follow-up skills
• Experience in skilled nursing, long-term care, hospital relations, admissions, or healthcare marketing preferred
