About the Role
The Hospital-Community RN Liaison accepts Hospice, Home Health, and Home Infusion referrals across various settings such as acute care, physician clinics, nursing homes, and rehabilitation centers. They maintain a strong presence in these environments to deliver exceptional customer service to discharge planners, social workers, physicians, and patients. Key duties include facilitating timely discharges, supporting continuity of care, and communicating services effectively to healthcare providers.
Requirements
- Registered Nurse license in applicable state (bachelor's degree preferred)
- Minimum three years of nursing experience; hospice and home health experience preferred
- Excellent observation, assessment, communication, and documentation skills
- Self-directed, flexible, and team-oriented
- Basic computer and electronic medical record skills
- Knowledge of current guidelines, regulations, and clinical standards
- Ability to determine patient appropriateness for services
Preferred Qualifications
- Certification in Hospice and Palliative Nursing (CHPN)
