Hospice Social Worker (MSW)

About the role

As a Hospice Medical Social Worker (MSW), you will be responsible for the provision of consultation and service to homebound individuals in need of assistance with the solution of complex social and emotional problems, which impede management of medical regimen.

Responsibilities

• Provide consultation and service to homebound individuals in need of assistance with the solution of complex social and emotional problems, which impede management of medical regimen.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Assess and address the psychosocial needs of patients and their families.
• Coordinate services with community resources to enhance patient care.
• Document all interactions and interventions in accordance with regulatory requirements.

Requirements

• Master's Degree from a school of social work accredited by the Council on Social Work Education.
• State of Michigan Full Masters Social Work License Required.
• Two (2) years of social work experience in health care setting.
• Ability to provide service to acutely and chronically ill patients.
• Ability to complete written, verbal, and observational tasks indigenous to the position.

Qualifications

• Must have a valid driver’s license and reliable transportation.
• Must be able to lift 50 pounds and perform physical tasks required for the job.

• Proficiency in Microsoft Office Suite.
