Hospice Social Worker (MSW)

Job Summary

At Brighton/Suncrest Hospice, we aim to enhance hospice care in your area through increased staff-to-patient ratios and improved clinical visits. We are CHAP certified and seek candidates committed to providing high-quality care.

What You'll Be Doing

• Assess, diagnose, and treat patients’ mental and social conditions
• Counsel individuals and families
• Plan and provide interventions
• Advise on available community resources
• Maintain client care records
• Attend and participate in client care conferences and staff meetings

Qualifications

• Thorough knowledge of social work principles and methods
• Knowledge of individuals and group behavior
• Ability to exercise independent professional judgment
• Effective communication skills
• Master’s degree in Social Work required
