Hospice Social Worker (MSW)

Role: Social Worker MSW Key Responsibilities Provide psychosocial assessments, counseling, and care planning for patients with life-limiting illnesses and their families.Coordinate resources and support in home and facility settings.Collaborate with an interdisciplinary team to advocate for patient dignity and address emotional and spiritual needs.Assist with advance care planning and ensure compassionate end-of-life care. Qualifications & Skills Possess a Master's in Social Work (MSW).Demonstrate compassion and strong interpersonal skills.Ability to assess psychosocial needs and develop appropriate care plans.Effective collaborative and advocacy skills.
