About the role
Provides emotional and psychosocial support to individuals and families, linking them to community resources based on ongoing assessments.
Responsibilities
- Plans, implements, and evaluates care
- Offers support during on-call hours
- Assesses psychosocial needs
- Maintains effective communication with team members, clients, and families
Requirements
- Master’s in Social Work from a CSWE-accredited program
- Current Social Work license in KY and/or OH
- Experience in healthcare, preferably with terminal illness, palliative care, home health, or bereavement
- Knowledge of medical terminology
- Must be able to access client homes independently
- Must observe universal precautions
- Must maintain valid insurance, driver’s license, reliable transportation, and pass background and drug screenings
