Hospice Medical Social Worker (Master's Level) (38365)

Responsibilities

• Conduct psychosocial assessments
• Provide counseling and emotional support to patients and families
• Aid with advanced care planning and coordination of community resources
• Participate in interdisciplinary team meetings
• Document services accurately and timely in the EMR HCHB

Qualifications

• Master’s degree in Social Work preferred
• Bachelor’s degree in Social Work accepted with required oversight
• Hospice experience preferred
• Experience using HCHB (Homecare Homebase) preferred
