Master Social Worker (Hospice)
Key Responsibilities
- Conduct comprehensive psychosocial assessments to identify patients' social, emotional, and environmental needs.
- Provide counseling, emotional support, and crisis intervention for hospice patients and their families.
- Educate patients and caregivers regarding disease progression, coping strategies, advance care planning, and available community resources.
- Develop and implement individualized care plans in collaboration with the interdisciplinary hospice team.
- Carefully coordinate referrals to community agencies and support services.
- Complete timely and accurate clinical documentation following each patient visit.
- Participate in interdisciplinary team meetings and contribute to patient care planning.
- Aid with bereavement services and caregiver support as needed.
- Function as a mentor and preceptor for newly hired social workers.
- Provide education to hospice staff on psychosocial issues and community resources.
- Aid in the development and revision of hospice social work and bereavement policies.
- Facilitate monthly caregiver support groups.
- Collaborate with hospitals and community agencies to ensure continuity of care.
- Travel to patient homes throughout the assigned service area.
Required Qualifications
- Master's Degree in Social Work (MSW) from a Council on Social Work Education (CSWE) accredited program.
- Minimum one (1) year of social work experience in a healthcare setting.
- Active Arkansas Social Worker License (or state-specific licensure as required).
- Current CPR Certification.
- Valid Driver's License.
- Reliable transportation with current auto insurance.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work independently while collaborating within an interdisciplinary team.
Preferred Qualifications
- Previous hospice or home health experience.
- Experience with bereavement counseling and end-of-life care.
- Knowledge of community resources and care coordination.
- Leadership, mentoring, or supervisory experience.
Work Environment
- Patient home visits throughout the assigned service area.
- Office based at Elite Hospice, Forrest City, AR.
- Scrubs or professional business attire required.
- Monday-Friday schedule with no holiday shifts.
