Hospice Social Worker (Masters Level)

About the role

The Medical Social Worker provides emotional and psychosocial support services and linkage to community resources for individuals and families.

Responsibilities

• Provides emotional and psychosocial support services and linkage to community resources for individuals and families.
• Plans, implements, and evaluates care through systematic assessment of psychosocial needs.
• Provides care and assistance to patients and families during on-call hours as scheduled.

Requirements

• Masters degree in Social Work from a CSWE-accredited program.
• Experience in a health care setting, prefer experience with terminal illness, palliative care or home health, and bereaved families.
• Current knowledge of medical terminology.
• Must have Social Work license in KY and/or OH.
• Personal qualities of empathy, responsibility, dependability and intelligence.
• Must demonstrate effective communication skills with team members, clients, and family members.
• Must have the ability to work independent of direct on-site supervision.
• Must demonstrate leadership and teaching ability.
• Must arrange travel and be able to access client homes independently.
• Must observe universal precautions.
• Must carry state minimum in automobile liability insurance, have a valid drivers license, reliable transportation, clear driving record, pass a drug screening and background check.
