About the role
As a Social Worker for Caring Hospice, you will provide quality hospice care, end-of-life, educational, and grief support services. Conduct hospice assessments and patient visits.
Responsibilities
- Conduct comprehensive hospice assessments and patient visits
- Provide grief counseling and support
- Assist with funeral arrangements, patient care planning, accessing community resources, and discharge planning
- Foster open communication with patients, patients’ families, facility partners, staff, and other healthcare providers
- Work effectively on an interdisciplinary team
Requirements
- Licensed Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in the state of New Jersey
- Minimum of 2 years of hospice or oncology experience preferred
- Previous experience in conducting comprehensive assessments
- Reliable transportation and ability to travel to patient residences
