About the role
Regional Hospice is seeking an experienced Social Worker (MSW preferred) to join our hospice interdisciplinary team serving the Lebanon, MO area. The ideal candidate will have some medical care experience and live within a commutable distance to the office.
Responsibilities
- Visit patients wherever they reside—private home, skilled nursing facility, assisted living facility, or hospital.
- Complete a psychosocial assessment on each assigned patient in a timely manner and document in the electronic medical record.
- Utilize community resources for patients, such as in-home services, Meals on Wheels, etc.
- Assist the patient, family, and caregiver in coping with issues related to death and dying and participate in grief support as needed.
- Provide information, complete hospice admission paperwork, and educate patients, families, and the community on all aspects of hospice services.
Requirements
- Graduated from a School of Social Work approved by the Council of Social Work Education with a Master’s Degree (preferred). Bachelor’s Degree in Social Work candidates will be considered. (Other degrees/areas of study do not meet state guidelines and will not be considered.)
- Flexibility with scheduling and hours is a must, as work hours are primarily Monday through Friday, 8am to 5pm, with the understanding a crisis can occur at any time.
- Computer skills for charting/EMR, basic use of Word, Excel, and Email; smart phone for mileage apps, etc.
- A loving, compassionate heart is essential.
Qualifications
- Some knowledge of bereavement is a strong plus.
- Willingness to train the right person.
Pay
Hourly rate based on years of experience, with mileage reimbursement provided. Recent company-wide increase in mileage.
Schedule
Primarily Monday through Friday, 8am to 5pm, with flexibility required for crisis situations that may occur at any time.
