Hospice Social Worker

About the role

As Social Worker for Caring Hospice, you will provide quality hospice care, end-of-life, educational and grief support services. You will conduct hospice assessments and patient visits.

Requirements

• Licensed Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in the state of MA
• Minimum of 2 years of hospice experience preferred in hospice or oncology
• Previous experience in conducting comprehensive assessments
• Ability to work effectively on an interdisciplinary team
• Prior experience making funeral arrangements; patient care planning, accessing community resources and discharge planning
• Ability to provide grief counseling/support
• Ability to foster open communication with patients, patient’s families, facility partners, staff and other healthcare providers
• Reliable transportation, and be able to travel to where the patient resides
