Responsibilities
- Assess patients’ psychosocial needs and complete social histories
- Document care plans, progress, and follow-up per regulations
- Provide counseling and emotional support to residents and families
- Support patients and families with adjustment, rights, and orientation
- Coordinate referrals and act as a liaison with community resources
- Partner with the care team to enhance quality of life and outcomes
- Plan and support patient, family, and community engagement activities
- Promote facility programs and services to referral partners
Requirements
- Bachelor’s Degree in Social Work, required
- Hospice or palliative care experience, preferred
- Two (2) years of verifiable experience working as a Social Worker, preferred
- Current driver’s license, automobile liability insurance, and the ability to travel between multiple locations within your region
