About the role

Responsibilities
- Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral
- Identifies and reassesses the patient's physical, psychosocial, and environmental needs at least every 15 days
- Develops and implements the patient's plan of care in collaboration with other healthcare professionals
- Collaborates with the patient, family, and medical team to provide care
- Instructs and supervises the patient and family in self-care techniques
- Maintains accurate clinical notes documenting the patient's condition and progress
Requirements
- Current and unrestricted RN licensure in the state of practice
- Current driver's license, valid vehicle insurance, and access to a reliable vehicle, or public transportation
Qualifications
- 1+ years of clinical experience preferred
- Current CPR certification or ability to obtain within 90 days of hire
Skills
- Familiarity with end-of-life care concepts and needs
- Strong communication and interpersonal skills
- Ability to work independently and manage time effectively

Pay
The salary for this role ranges from $79,600 to $119,400 annually, based on full-time employment. Other benefits include a drug-free workplace policy and a commitment to mitigating environmental impact and improving health outcomes.
Schedule
Full-time position with standard business hours of Monday-Friday, 8:00 AM-4:30 PM. Oncall rotation for evenings and weekends as needed.
Location
The agency serves a 50-mile radius, and candidates must be willing and able to travel throughout the coverage area to provide patient care.
