About the role
The Registered Nurse Case Manager is responsible for delivering comprehensive hospice care by developing and implementing personalized Plans of Care in collaboration with patients, families, and healthcare professionals.
Responsibilities
- Conducting initial and ongoing assessments
- Identifying patient needs
- Documenting care activities
- Coordinating treatment strategies that address physical, psychosocial, emotional, and spiritual well-being
- Providing patient and family education
- Supervising self-care techniques
- Maintaining accurate clinical documentation
Qualifications
- Current, unrestricted RN license
- Valid driver's license
- Reliable transportation
Preferred qualifications
- At least one year of clinical nursing experience
- CPR certification
Pay
$71,640 to $107,460 annually
