Responsibilities
- Provides professional nursing care by utilizing all elements of the nursing process.
- Assesses and evaluates patient/client status by:
- a. Reviewing and implementing the Plan of Care (POC)
- b. Regularly re-evaluating patient/client needs
- c. Revising the POC as necessary
- Administers medications and treatments as prescribed by the physician.
- Initiates and applies appropriate preventative, therapeutic, and rehabilitative nursing procedures and techniques and maintains communication regarding patient/client care with the physician and supervisory personnel.
- Regularly observes the patient/client for changes in condition, discusses changes with the patient/client or family/caregiver, and reports changes, as necessary, to the attending physician.
- Affords assistance to the patient/client with the activities of daily living and facilitates the patients/clients effort toward self-sufficiency and independence.
- Teaches the patient/client and family/caregiver self-care techniques as appropriate. Provides medication, diet, and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients/clients and families/caregivers.
- Provides and maintains a safe environment for the patient/client.
Qualifications
- Currently licensed to practice as a RN in the state of practice.
- If IV or ET certified, is currently certified and recognized by the state(s) where currently practicing as an RN.
- Possesses excellent observation, good nursing judgment, and good oral and written communication skills.
- Possesses and maintains good physical stamina and mental health.
- Presents a pre-employment physician’s health clearance, including negative TB skin tests and/or CXR and other tests as required by specific state statutes.
- Possesses and maintains current CPR certification.
- Is a licensed driver and has an automobile that is insured in accordance with Lorian Health (LORIAN) requirements.
- Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
