Hospice RN

About the role

This position involves evaluating clients, developing personalized care plans, and providing preventive and rehabilitative nursing services to support hospice patients and their families. The nurse collaborates with an interdisciplinary team, assesses patient needs, and promotes respectful, culturally sensitive care. Responsibilities include monitoring patient conditions, updating care plans, and ensuring quality outcomes through evidence-based practices. When acting as a case manager, the nurse coordinates all aspects of patient care. This role is a safety-sensitive position focused on delivering compassionate, patient-centered hospice care in a home setting.

Responsibilities

• Assess patient preferences and barriers to involvement in care
• Develop, implement, and document individualized care plans
• Communicate effectively with patients, families, and team members
• Evaluate patient progress and update care strategies accordingly

Qualifications

• Valid RN license and driver's license
• Minimum of 2 years nursing experience; experience in hospice preferred
• Strong assessment, communication, and critical thinking skills
• Ability to work safely and maintain alertness
