Hospice RN

Role Overview This is a full-time nursing position supporting hospice patients and their families within the community. The role involves evaluating patients, developing individualized care plans, and coordinating services to promote patient dignity, comfort, and quality of life. Key Responsibilities Assess patient preferences, needs, and barriers across emotional, spiritual, and cultural domains.Implement and document personalized care plans in collaboration with the healthcare team, patients, and families.Promote respectful and equitable care, supporting diverse populations through empathetic interactions.Evaluate patient condition changes, communicate updates, and adjust care plans accordingly.Ensure evidence-based practices and quality outcomes, maintaining safety as a priority. Qualifications & Skills Licensed RN with a valid driver's license.Associate's Degree in Nursing required; Bachelor's preferred.Minimum of 2 years of nursing experience; experience in hospice or home health is advantageous.Strong clinical assessment, communication, and interprofessional collaboration skills.Ability to work independently, demonstrate critical thinking, and prioritize patient-centered care.
