Hospice Resource STNA - 2PM - 10:30 PM

About the role

This is a full-time role that runs from 2PM to 10:30PM. You will work under the direction of a Case Manager/DON and provide resident care to assigned residents according to assessments, care plans, and as directed by nursing management. You will also coordinate care with other licensed care staff and follow Company nursing policies and procedures, as well as safety guidelines.

Responsibilities

• Provide resident care in accordance with resident assessments, care plans, and as directed by nursing management.
• Coordinate care with other licensed care staff.
• Follow Company nursing policies and procedures as well as safety guidelines.
• Work closely with patients on a 1-1 level.
• Get strong support from a patient care team.
• Work independently and on your own from a day-to-day basis.

Requirements

• A strong commitment to improving quality of life while partnering with community organizations in patient care.
• The ability to work well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and STNA leadership.
• An eagerness to gain higher level opportunities such as certifications and further education.
• Able to work independently and on your own from a day-to-day basis.

Qualifications

• A State Tested Nursing Assistant certification in the state(s) of practice.
• Willingness to drive with reliable transportation, valid driver’s license, and auto insurance.
