Hospice Resource STNA - 2PM - 10:30 PM

About the role

This role involves working under the direction of a Case Manager/DON to provide resident care to assigned residents in accordance with resident assessments, care plans, and as directed by nursing management. Responsibilities also include coordinating care with other licensed care staff and adhering to company policies and safety guidelines.

Responsibilities

• Provide resident care in accordance with resident assessments and care plans
• Coordinate care with other licensed care staff
• Adhere to company nursing policies and procedures
• Follow safety guidelines
• Work closely with patients on a 1-1 level
• Get support from a patient care team
• Work independently and on your own from a day-to-day basis

Requirements

• Strong commitment to improving quality of life while partnering with community organizations in patient care
• Ability to work well within a team environment to collaborate with nurses, social workers, chaplains, volunteers, and STNA leadership
• Eager to gain higher level of opportunities such as certifications and further education
• Able to work independently and on your own from a day-to-day basis
• Willingness to drive with reliable transportation, valid driver’s license, and auto insurance
• State Tested Nursing Assistant certification in the state(s) of practice
