About the Role

Provide personal healthcare and related services to patients in their homes, assisting with safe transfers, ambulation, nutrition, and hygiene under the supervision of a registered nurse.

Responsibilities

• Document patient observations and report changes in condition
• Maintain a clean and safe environment for patients
• Promote respectful communication with patients and families
• Perform treatments as per agency policies and procedures
• Ensure infection control and support patient comfort and dignity
• Assist with mobility, transfers, and ambulation
• Provide proper nutrition and hygiene care
• Attend ongoing training and adhere to organizational policies

Qualifications & Skills

• High school diploma preferred
• Valid driver's license required
• At least one year of experience as a Home Health Aide or Nursing Assistant preferred
• Completion of a state-approved training and competency evaluation program required
• Proven ability to read, write, and verbally communicate clinical information
• Current CPR certification required
• Reliable transportation required

Physical & Environmental Conditions

• Prolonged standing and walking
• Lifting up to 50 pounds
• Travel to patients' homes in various conditions
• Exposure to infectious materials and weather challenges
• Flexibility and manual dexterity essential
