Hospice Registered Nurse

About the Role

Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency, and assume responsibility for coordination of care in North County / North City.

Responsibilities

• Perform initial and ongoing comprehensive assessments of the impact of terminal disease on patients' physical, functional, psychosocial, and environmental needs, including ADLs, grief risk, cultural/spiritual, and verbal/non-verbal communication.
• Implement and revise the individualized Plan of Care (POC) as necessary.
• Consult with and educate patients/families regarding disease processes, self-care techniques, end-of-life care, nutrition, and dietary needs.
• Provide training to other staff as needed.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Prepare clinical and progress notes demonstrating progress toward established goals.
• Coordinate all patient/family services and prioritize needs with members of the Interdisciplinary Team (IDT).
• Use a case management approach and refer to other services as needed.
• Inform physicians and other personnel of changes in patient needs and outcomes of interventions, while evaluating patient/family response to care.
• Determine the scope and frequency of services based on acuity and patient/family needs.
• Assess the caregiver’s ability to meet the patient’s immediate needs upon admission and throughout care.
• Evaluate personal support needs and use identified systems to meet them.
• Communicate information using current processes and technology available to the organization.
• Apply specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and patient eligibility.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Other duties as assigned.

Qualifications

• Graduate of an accredited Associate or Bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; previous hospice or home health experience preferred.
• Current CPR certification.
• Proficient in using a computer; Microsoft Office suite experience preferred.
• Strong interpersonal skills and ability to provide effective support to patients and families.
• Demonstrated skill in nursing practice, ability to cope with family emotional stress, and tolerance of individual lifestyles.
• Strong communication, organizational skills, and high standards of ethics and compliance.
