Hospice Registered Nurse

About the Role

Deliver and evaluate personalized care plans for patients facing end-of-life in a hospice care setting.

Responsibilities

• Conduct initial nursing evaluations within 48 hours of referral
• Assess and reassess patients' physical, psychosocial, and environmental needs
• Develop and coordinate individualized care plans
• Document patient progress and maintain accurate clinical notes
• Collaborate with patients, families, physicians, and care teams
• Educate families on self-care techniques
• Adapt care to diverse home situations

Requirements

• Current RN license in the state of practice
• Valid driver’s license and reliable transportation

Preferred Qualifications

• 1+ years of clinical experience
• CPR certification

Pay

Salary ranges from $87,560 to $131,340 annually.
