Hospice Promise is seeking a dedicated team member to join our hospice care team. The ideal candidate will have a compassionate heart and a strong desire to provide end-of-life care.

Responsibilities

• Provide comprehensive hospice care services to patients and their families
• Collaborate with interdisciplinary team members to develop and implement care plans
• Offer emotional, spiritual, and practical support to patients and families
• Document patient care in accordance with regulatory requirements

Requirements

• Bachelor's degree in a related field (e.g., nursing, social work)
• Current license to practice in your state
• Experience in hospice or palliative care preferred
• Excellent communication and interpersonal skills
