About The Job

A Hug Away Healthcare is a small hospice agency in Houston, TX and surrounding areas. We are seeking a Hospice Liaison to help grow our patient base through relationships with healthcare providers and community organizations.

Key Responsibilities

• Develop and maintain relationships with healthcare providers, hospitals, nursing homes, and other potential referral sources.
• Identify and develop new accounts to increase patient referrals.
• Meet and exceed established quotas for patient admissions.
• Structure and grow designated areas of Houston to ensure consistent growth and service delivery.
• Conduct presentations and educational sessions to promote hospice services.
• Collaborate with the clinical team to ensure seamless patient care and service delivery.
• Maintain and report on market trends, competitor activities, and potential opportunities for growth.
• Ensure there are no service failures by maintaining high standards of communication and follow-up.
• Work some weekends as required to meet the needs of the agency and its patients.

Qualifications

• Minimum of 2 years of experience in a hospice liaison or similar role.
• Established relationships with nurses, case managers, and other sources for families we can help.
• Proven ability to meet and exceed quotas.
• Existing book of business and ability to hit the running.
• Strong understanding of the hospice industry, including compliance and regulatory requirements.
• Excellent communication, presentation, and interpersonal skills.
• Ability to work independently and as part of a team.
• Strong organizational and time management skills.
• Able to quickly build territories.
• Valid driver's license and reliable transportation.
