Hospice - LVN Clinical Coordinator

Patient Care Coordination

• Serve as a key liaison between patients, families, physicians, and hospice clinicians.
• Carefully coordinate care services and assist with resolving patient care concerns.
• Support communication across the interdisciplinary team.

Clinical Documentation & Orders

• Request, track, clarify, and process physician orders.
• Upload and manage documentation within the EMR system.
• Ensure records are complete, accurate, and compliant.
• Aid in clinical documentation and workflow management.

Clinical Operations

• Support field clinicians with scheduling and care coordination.
• Maintain and monitor medical supply inventory.
• Ensure the supply room remains organized and OSHA compliant.
• Aid in audits, quality improvement initiatives, and regulatory readiness.

Team Support

• Aid in onboarding and orienting new team members.
• Collaborate with nurses, social workers, chaplains, physicians, and leadership.
• Contribute to a positive and supportive team culture.

Qualifications

• Current California LVN License in good standing.
• Strong communication and customer service skills.
• Excellent organizational and multitasking abilities.
• Ability to work independently and prioritize responsibilities.
• Knowledge of medical terminology.
• Proficiency with computers and electronic medical records.
