Hospice LNA
Key Duties
- Performs basic nursing procedures as indicated in competency assessment to include but not limited to taking and recording of vital signs, finger stick PT INR, simple, non sterile dressing changes, bed bath, sponge tub and shower bath, shampooing, nail and skin care, oral hygiene, toileting and elimination, nutrition and fluid intake, application of ointments, creams and topicals, reporting on skin conditions within State of New Hampshire LNA Scope of Practice.
- Performs tasks and provides personal care to patients following the plan of care in a safe and effective manner complying with infection prevention and control policies.
- Makes transfers, performs range of motion, positioning, exercises, assists with activities of daily living (ADLs), ambulation, functional activities and other basic nursing care as instructed.
- Completes competencies in one of the following areas: (1) finger stick PT INR, phlebotomy/lab draws; or (2) advanced wound care as instructed by RN and allowed by LNA scope of practice.
- Accurately and timely documents patient care information on appropriate patient medical record forms and/or electronically.
- Facilitates professional and therapeutic relationships with all patients, families, and health care professionals.
- Maintains open communication with patient, patient representative, caregiver, and family and honors patient rights.
Education Requirements
- High School Diploma or equivalent
- RHIT or equivalent preferred
- Experience in medical records management required

Pay
The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
