LPN LVN - Hospice

About the role

This position will be traveling to see hospice patients in the Columbia Gorge service area. The schedule is weekdays day shift with rotating weekend coverage.

Responsibilities

• Works under the direction of a registered nurse to provide quality nursing care to patients in their homes.
• Aids in the collection of assessment data and contributes to the planning of care.
• Implements the care plan within the scope of licensure.
• Performs duties in a manner that promotes teamwork and reflects the Providence mission and philosophy.

Requirements

• Graduate of an accredited LPN/LVN school of nursing.
• Within 30 days of hire: National Provider BLS - American Heart Association.
• Upon hire: Oregon Licensed Practical Nurse.
• Upon hire: Washington Licensed Practical Nurse.
• Driving is a necessary part of this role. Caregivers must comply with all state laws and requirements for driving. Proof of driver license and auto insurance is required upon request.
• Experience in nursing; long term care, acute care, ambulatory care or home health and hospice care (1 year preferred).

Qualifications

Required qualifications include:

Pay

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. The base pay range is $31.35 - $43.33.
