LPN/LVN - Hospice

Role Overview

This position involves traveling within the Columbia Gorge area to provide in-home hospice nursing care under the supervision of a registered nurse.

Key Responsibilities

• Assist with patient assessments and contribute to care planning
• Implement care plans within licensure scope
• Deliver compassionate, team-oriented care reflecting the organization's mission
• Perform duties in patients' homes, ensuring quality and safety

Qualifications & Skills

• Graduate from an accredited LPN/LVN program
• Licensed Practical Nurse license in Oregon and Washington
• CPR certification within 30 days
• One year of nursing experience in long-term, acute, ambulatory, home health, or hospice care
• Valid driver's license and auto insurance, with willingness to travel

Preferred Experience

• Home health, hospice, or public health experience
