About the role
This role involves providing in-home hospice nursing care within the Columbia Gorge area, working under the supervision of a registered nurse.
Responsibilities
- Collect assessment data
- Assist in care planning
- Implement care plans within licensure scope
- Promote teamwork and alignment with the organization’s mission
Requirements
- Graduation from an accredited LPN/LVN program
- Active Oregon and Washington LPN licenses
- At least one year of nursing experience in settings such as home health or hospice
- Strong communication and patient assessment skills
- Adherence to safety protocols, including driving responsibilities
