Hospice Liaison (Patient Advocate)
What you will do
- Build and manage a referral territory — cultivating relationships with physicians, hospital case managers, SNF social workers, and other healthcare partners
- Educate referral sources on Ovation's clinical capabilities, patient criteria, and the value of timely hospice referrals
- Meet with families to explain hospice benefits and help them understand their options during an emotionally complex time
- Drive admissions growth by consistently generating qualified referrals and converting them to patients in care
- Maintain accurate activity tracking in CRM — logging visits, contacts, and pipeline with discipline
- Collaborate closely with your Administrator, clinical team, and intake coordinators to ensure seamless hand-offs and patient experience
- Represent Ovation in the community with professionalism, humility, and genuine care for the people you serve
What we are looking for
We are not just screening for a resume. We are looking for a specific kind of person. The right candidate will recognize themselves in most of the following:
- You build trust quickly — in a hospital hallway, a SNF conference room, or a family's living room, you know how to show up and earn credibility fast
- You are self-directed — you plan your own week, work your own territory, and do not need someone to tell you where to be or when to follow up
- You are resilient — a cold week or a lost referral does not derail you; you reset and keep going
- You are motivated to win — you track your own numbers, you know where you stand, and you care about hitting your goals
- You are aligned with the mission — you understand that hospice care is a gift, and you feel the weight and the honor of being the person who helps families access it
COMPENSATION AND BENEFITS
- Competitive Base Salary: $70,000 - $90,000
- Competitive Bonus Plan: Performance-based
- Mileage Stipend and Cell Phone Reimbursement
- Comprehensive Benefits: Medical, dental, vision, PTO, 401(k) retirement plan
About Ovation Hospice
Ovation Hospice is a multi-market hospice provider operating in Phoenix, Denver, and Salt Lake City. We are building something intentional — a company where clinical excellence, operational discipline, and genuine culture are not in tension with each other. We believe great care starts with the right people, supported by strong systems and real leadership.
Hiring Process
We run a structured, intentional process — out of respect for your time and ours. Every candidate goes through the same stages:
- Written application question
- Phone screen with a market leader
- Structured in-person interview
- Short behavioral assessment
- Field ride with an Ovation leader (2–3 hours)
- Final hiring decision
The field ride is mandatory and is one of the most important parts of our process. We want to see how you show up in the real environments where this work happens — and we want you to see what you are walking into. This is as much for you as it is for us.
Ready to Apply?
Complete the application and the prescreen survey that will be sent to you. This is important as we usually only review applicants who complete the survey. Instead of a cover letter, please submit a brief response to the following question: "Describe a time you had to build a relationship from scratch in a difficult environment. What did you do?"
